WHAT YOU CAN EXPECT FROM US?
Priority's Support Services offer support before, during and after your move into accommodation. Support is offered until you feel confident that you can live independently. At the time of moving into a new home, lots of support and advice will be given. As your confidence grows you will find that you will need your Support Worker less.
NEEDS ASSESSMENT
After your initial interview with Priority your Support Worker will provide you with a copy of your written "Needs Assessment". This will be based on your interview with Priority and will also take into account any additional information (for example your application for support).
The Needs Assessment will describe the main areas of support that have been identified by you and the staff carrying out the interview. Priority will provide some of the support you need - other agencies may also be involved in providing support as well.
SUPPORT AGREEMENT
When support starts you and your Support Worker will complete a Support Agreement. This sets out the support that you can expect from Priority, and your responsibilities as a service user.
SUPPORT PLANS
You and your Support Worker will complete your Support Plan soon after support starts, usually within one month of support starting.
Your Support Plan will set goals that are relevant to you, and set out how these goals might be achieved. It will look at areas of work which need to be covered by you, your Support Worker, and anyone else that may be supporting your move into independent accommodation.
Your Support Plan will be reviewed every 6 months, or sooner if there is a significant change in your circumstances. You have the right to ask for your Support plan to be reviewed at any time.
COMPLAINTS
AND APPEALS
You
can appeal against any decision relating to your Needs Assessment,
Support Plan or Support Plan review, and have the right to
be accompanied at the appeal meeting. For further details
please read about "Making a Complaint" in this booklet.
WHAT PRIORITY
CAN HELP WITH:
- Support with Housing
Benefit and other benefit claims.
- Practical help, especially
setting up home.
- Help with practical
skills such as budgeting and cooking.
- Visiting you after
you have moved into a new home, for up to 2 years, until
you feel confident that you can live independently.
- Finding out about
training, work and education opportunities, community facilities
(e.g. sports and leisure), and other activities that interest
you.
- Help with finding
out about counselling or support from other agencies.
WHAT IS MY SUPPORT WORKER
RESPONSIBLE FOR?
- Providing
appropriate support to assist you to live independently
of Priority, as set out in your Support
Plan.
- Meeting with you regularly
as set out in your Support Plan
- Discussing your
responsibilities as a tenant or licensee.
WHAT AM I RESPONSIBLE FOR?
- Accepting support
from your Support Worker, and working with him/her to develop
the skills you need to live independently in the community
- Being committed to
working to a planned programme of support, as set out in
your Support Plan. THIS INCLUDES KEEPING APPOINTMENTS MADE
BY YOUR SUPPORT WORKER.
- Observing the conditions
of your tenancy/licence agreement
- Being
committed to paying rent regularly and on time. This includes
making sure that any housing benefit claim is maintained.
Also paying your fuel, water rates, TV licence and council
tax when appropriate.
